The home fire safety booking form is not fully accessible due to the following reasons:
Heading elements are not used consistently to convey the structure of the form. This fails WCAG success criteria 1.3.1 (Info and relationships).
Some form fields are missing programmatic labels, or the labels are not associated with the interactive page element, so the labels are not announced in a useful way by a screen reader. This fails WCAG success criteria 1.3.1 (Info and relationships), 3.3.2 (Labels or Instructions) and 4.1.2 (Name, Role, Value).
The colour contrast of error messaging is insufficient in places making it hard from some people to read content. This fails WCAG success criterion 1.4.3 (Contrast (Minimum))
The focus order through the form does not preserve meaning. This fails WCAG success criterion 2.4.3 (Focus Order).
The form is embedded as iframes and is missing a clear title so it may not be clear to screen reader users what is contained within the form. This fails WCAG success criterion 4.1.2 (Name, Role, Value).
We are working in partnership with our fellow Fire and Rescue Services to develop a new, accessible home fire safety booking form. In the meantime, anyone wishing to book a home fire safety check who has trouble accessing the form is encouraged to contact us by phone.
